COMING THIS YEAR! As all of you know, taking action is proof of resolving ambivalence. Making a public declaration of what this action will be is a further commitment step. So, it that spirit, I am announcing that I am taking the initial steps to create a series of MI demonstration DVDs and a MI book/ digest/practice guide. Both of these are intended to supplement the already existing excellent MI text and training videos. I'll let you know when these are ready!
ABOUT MY WORKSHOPS: I have provided MI training since 1995, following my training with Drs. Miller and Rollnick. My teaching approach is very pragmatic, focusing on the core concepts of MI and allowing the learning to progress from direct practice.
I have elected to limit the size of my workshops in order to maximize the opportunity to provide learning that can both address the concepts and practice of Motivational Interviewing while at the same time assist in the application of MI to each participant's work setting.
I offer all participants the opportunity to submit a 15-minute audio-tape of an interview using the MI approach before and after each workshop. This will allow for you to have feedback on your progress in learning.
Onsite training is limited to 15 participants, both in Advanced and Introductory MI; Distance Learning classes are limited to 10.

Welcome to Motivation News, a newsletter devoted to clinical information on the use of a Motivational Interviewing (MI) approach to change. The authors of this approach, William Miller, PhD and Stephen Rollnick, PhD define MI as a 'client centered, directive method for enhancing an intrinsic motivation for change by exploring and resolving ambivalence". This collaborative approach to working with clients has wide application in coaching, mental health, addictions, health care and health education. Information in this newsletter is taken from various source material from the Motivational Interviewing Network of Trainers and specifically from the book Motivational Interviewing, Preparing People for Change; Miller and Rollnick, 2nd edition, 2002, Guilford Press.
AUGUST MAIN FEATURE
CONTINUING WITH INCREASING IMPORTANCE
When a person is low on importance, we are using OARS and Rolling with Resistance to develop discrepancy by focusing on DESIRE to change, ABILITY to change, REASONS to change, and NEED to change.
In the middle phase of importance where the scales are more balanced around making change, we continue to use OARS and add Methods of Eliciting Change Talk, attending to reflecting change talk as we hear it from the client. The goal of this phase is to resolve the ambivalence about change and move toward a change plan if the client has progressed that far.
Last month we interviewed a person who is seeing a health behavior change professional based on a referral from a physician on weight loss. She was initially on the low end of importance. The discussion went like this: (I) is for interviewer, (C) is for client.
I: ‘Hi Mrs. Jones; Dr. Smith asked me to talk with you about losing some weight.’ (agenda setting)
C: ‘I know that losing weight is important.’ (You may be thinking ‘great, no ambivalence here!’)
I: ‘What are some reasons for losing weight?’ (open question)
C: ‘Well, not being so heavy helps with back and knee pain, reducing the possibility of diabetes and I think for women being overweight increases risk for breast cancer. (reasons)
I: ‘So you believe that it is important for you to lose some weight.’ (reflection)
C: ‘I’m not saying that!’ (Oh, there is ambivalence!)
I: ‘Tell me more.’ (open question)
C: ‘Well, I just think the women in my family are heavy and I’m like them; I don’t think I’ll have any difficulties myself.’ (resistance)
I: ‘Okay, so while you can see the value of weight reduction for others, right now you don’t see the need for yourself.’ (reflection that rolls with resistance)
C: ‘Right.’
I: ‘Well, if losing weight did become more important to you, what would increase that importance?’ (open question)
C: ‘If it restricted what I want to do, seemed to interfere with my getting dates, needing to buy a larger size in my clothes, was causing some health problems, but I don’t see any of that now.’
I: ‘Okay, so what is your understanding of why your doctor wanted you to work on this?’ (open question)
C: ‘Well, she says that prevention is important, that if I wait ‘til I have problems, some damage could have occurred already; that makes sense in some ways; it just seems like a hassle to do something when there is no reason staring me in the face!’ (resistance, but not as strong)
I: ‘It doesn’t seem urgent, pressing right now even though you get what the doctor is saying and yet you were willing to talk to me about this.’ (double sided reflection)
C: ‘Sure, you can’t miss all this emphasis on how much you weigh; it’s in magazines, the newspaper, the internet, on tv; so even though I don’t want to pay attention to it, I do wonder should I do something.’
I: ‘You are not entirely against some consideration of doing something.’ (reflection)
C: ‘I am willing to make some changes.’ (ambivalence is tipping)
I: ‘And what would you be wiling to do?’ (open question that picks up on change talk)
As we ended last month, the ambivalence has started to shift. Let's pick it up here.
C: 'Well, at my church there is a project to encourage us to eat more fruits and vegetables, likely of the healthy kind! I had kind of thought about signing up for that.'
I: 'Tell me more of what you know about the project, the goals.' (open question)
C: 'It is really a part of our overall spirituality, treating our bodies well. I think the idea is that we will be healthier, living more with God's principles, if we take care of ourselves with how we eat.'
I: 'Doing this would be in keeping with your spiritual values.' (complex reflection)
C: 'It would and probably would be good for me to learn how to eat healthier; I assume I might lose some weight that way.'
I: 'You have reasons that connect with your values and a way to go about it and even before your doctor mentioned this, you had wondered if you should do something.' (summary that focuses on change) 'What do you think you will do?' (open question about a plan)
C: 'I have the phone number for the contact person for the church project and she is a friend of mine. I could call her.'
I: 'I hear you moving closer to making a change here; where would you rate yourself on readiness on a scale of 0-10 where 0 is not at all ready and 10 is as ready as I can be?' (use of one part of the ruler exercise)
C: 'Well, when we started talking, I was at a 3 or 4, but now that I think about this with all the others at church who will be doing it, I'd say I am an 8. After all, there is strength in numbers.'
I: 'You have found a way to make this personal for you.' (complex reflection)
C: 'Yes, I am going to call her tonight and tell her to sign me up!'
So now this client has moved from low importance to a plan.
Keep practicing!
