COMING THIS YEAR! As all of you know, taking action is
proof of resolving ambivalence. Making a public declaration
of what this action will be is a further commitment step.
So, it that spirit, I am announcing that I am taking the initial
steps to create a series of MI demonstration DVDs and a MI book/
digest/practice guide. Both of these are intended to supplement
the already existing excellent MI text and training videos. I'll let you
know when these are ready!
ABOUT MY WORKSHOPS:
I have provided MI training since 1995, following my training
with Drs. Miller and Rollnick. My teaching approach is very
pragmatic, focusing on the core concepts of MI and allowing
the learning to progress from direct practice.
I have elected to limit the size of my workshops in order to
maximize the opportunity to provide learning that can both
address the concepts and practice of Motivational Interviewing
while at the same time assist in the application of MI to each
participant's work setting.
I offer all participants the opportunity to submit a 15-minute
audio-tape of an interview using the MI approach before and
after each workshop. This will allow for you to have feedback
on your progress in learning.
Onsite training is limited to 15 participants, both in Advanced and
Introductory MI; Distance Learning classes are limited to 10.
This letter may be forwarded to anyone you feel could benefit
as long as it is forwarded in its entirety.

Welcome to Motivation News, a newsletter devoted to clinical
information on the use of a Motivational Interviewing (MI)
approach to change. The authors of this approach, William
Miller, PhD and Stephen Rollnick, PhD define MI as a 'client
centered, directive method for enhancing an intrinsic motivation
for change by exploring and resolving ambivalence\". This
collaborative approach to working with clients has wide application
in coaching, mental health, addictions, health care and health
education. Information in this newsletter is taken from various
source material from the Motivational Interviewing Network of
Trainers and specifically from the book Motivational Interviewing,
Preparing People for Change; Miller and Rollnick, 2nd edition,
2002, Guilford Press.
MAY MAIN FEATURE
STAGE TWO OF MI: STRENGTHENING COMMITMENT TO CHANGE
Once a client has resolved ambivalence about making change, it is time to
assist the client in clearly defining a change plan, as happened in our
example
from last month:
I: ‘So where are you now in how you might want to proceed?' (open question)
C: ‘Well, I have looked at a plan; the online Weight Watchers site and it is
easy to join and develop a plan for what to eat each day, even if I don’t cook
all the time; there is also a way to record my weight loss, keep track of
things. I can’t go to those meetings, won’t work for me, but I could let me
girlfriend see the plan, ask her to support it without trying to take it over. If I
can look at the plan for eating each day and check off each time I did it, that
would get me more organized. That is what I did to get the business going.’
I: ‘That is a really specific plan; what would help you stay consistent with this
plan, to be able to focus on the reason for this in relationship to those values
you outlined?' (open question to strengthen commitment)
C: ‘I have a picture of my family from our last Christmas gathering; I think I'll
put that on my refrigerator and have a copy on my desk at my office; that will
help me remember I have to stay healthy in order to be around for them.!’
I: 'Sounds like a plan; I'll look forward to hearing how it is going.'
in follow up, we do just exactly that, follow up to see how it is going, help the
client
problem solve and unanticipated barriers, look ahead to any challenges that
might be on the horizon, listen for any possible new ambivalence and
explore that.
Here is our check in at the next appointment (either by phone or in person,
according to your setting/research protocol). C is for client and I is for
interviewer.
I: 'Harry, it's Cathy from the Healthy Lifestyles Study calling for our scheduled
check in; is this still a good time to talk?' (agenda setting)
C: 'Yes it is and I have success to report!'
I: 'You sound pleased; tell me about it.'
C: 'Well, we agreed to give me two months to get going with the Weight
Watchers plan and I am pleased to report that I have lost 10 pounds.'
I: 'You made a plan and stuck to it!' (affirmation)
C: 'Yes, and I feel better already, kinda surprising. Once I lost the first five
pounds,
I decided to add some exercise, since you get more to eat that way on the
plan! So,
I started walking to my office on the days that i knew I did not need the car;
that
means I can walk about 4 miles, 2 each way.'
I: 'So you only committed to the change in eating and you then added
exercise, as well.' (affirmation; notice how this is a supporting statement but
does
not involve my praise; this is about the client's praise of self, not my praise of
client
and I just want to support that. The praise can imply that our relationship is
contingent on his success.)
C: 'Right, but I am a little concerned about the family reunion next week, that
big old cookout with lots of great desserts! I want to enjoy myself and not
gain
any weight.'
I: 'Okay, you are anticipating a BIG challenge! What ideas do you have on how
to meet your goal of enjoyment and not losing ground on your progress.'
C: 'Well, on the WW sight, they talk about how to handle those things: allow
yourself some treats that day and also do some things to be better prepared:
don't come completely famished, drink water while eating to feel fuller, take
smaller portions and allow yourself to not eat all of it.'
I: 'So there are some ideas from WW; what do you think you will do?' (did not
assume what he learned would actually be HIS plan)
C: 'Well, I don't know if I told you but my girlfriend is really trim, never seems
to
gain weight and can eat everything in sight, at least for now! So she and I
talked
about it...she is really excited for me with the 10 pound loss....and we are
going to
work together on this. We plan to walk the 3 miles from the hotel to the park
where the reunion is held so I'll get exercise both ways. Then I realize lots of
the
food I can eat just fine, the food that is barbequed....it's the desserts and side
dishes
that get me. Michelle is going to get those things on her plate and I'll get the
meat.
That way I can taste off her plate but not have all of it as mine. She always
takes
a lunch bag with things she likes to eat so I'm going to have foods I know I
can have to feel full and avoid the temptation of just chowing down. And, on
the WW site they
encourage you not to worry if you gain a pound or two, that it will come off
quickly if you return to the planned way of eating when you get home. You
know,
I realize that mostly the reunion is about seeing my family, so I can enjoy the
food
but I really want to spend time having fun with them.' (back to values!)
I: 'You have given it lots of thought and have a really specific
plan' (affirmation)
Anything else before we end the call today?'
C: 'No, I plan to tell you how much more weight I have lost when we talk next
time, 2 months, right?'
I: 'That's correct; I'll look forward to it.'
Keep practicing!
