Welcome to Motivation News, a newsletter devoted to clinical information on the use of a Motivational Interviewing (MI) approach to change. The authors of this approach, William Miller, PhD and Stephen Rollnick, PhD define MI as a 'client centered, directive method for enhancing an intrinsic motivation for change by exploring and resolving ambivalence\". This collaborative approach to working with clients has wide application in coaching, mental health, addictions, health care and health education. Information in this newsletter is taken from various source material from the Motivational Interviewing Network of Trainers and specifically from the book Motivational Interviewing, Preparing People for Change; Miller and Rollnick, 2nd edition, 2002, Guilford Press.
INCREASING IMPORTANCE
I'd venture to say that most of us do not go about deciding to make changes without some thoughts on it being important to change. So, what increases IMPORTANCE?
If I am going along my merry way, living my life as I want and without any significant distress, why would I WANT to make any changes? Unless I am having some inner dialogue about how I am and how I would rather be, likely I am not thinking of any changes.
IMPORTANCE often increases based on some external forces: impact on health, a spouse setting limits on behavior, or a boss saying shape up or ship out! Even then, we know of countless examples of this still not being a motivator for change.
Change and the recognition of the importance of change more likely occur when people are provided an open, non coercive opportunity to address the WHY of change. That is the point of MI; allowing people to address the importance of change, with the freedom to make choices.
Here is an example of how this might go:
Client (C): My diabetes is not that much out of control; I know others who do a lot worse than me!
Interviewer (I): You are thinking how things are is okay.
C: Well, I don't know if it is okay but it's not that bad. I can cheat and not get into too much trouble.
I: What would need to happen for you to see better control of your sugar levels as important?
C: Hmmm...if the doctor told me my sight was being affected or if I got a cut and it would not heal.
I: Fairly strong medical problems would need to happen in order to get your attention.
C: I wouldn't want to go blind or have an amputation! Will that happen if I cheat and have some high sugars?
I: Those extreme medical outcomes are scary to you.
C: Yeah, but it's a pain to be so strict in the way I eat.
I: On the one hand you want to eat as you choose and on the other hand you want to avoid problems.
C: If I just knew how much cheating would be okay!
I: A tough spot...wanting to have both things..eating and control.
C: Well, I don't want to be sick from this, have all that bad stuff. I guess I should think about not cheating so much.
I: It's feeling more important to you....making some changes to avoid bigger problems in the future.
And so the conversation continues.
Keep practicing!
