Welcome to Motivation News, a newsletter devoted to clinical information on the use of a Motivational Interviewing (MI) approach to change. The authors of this approach, William Miller, PhD and Stephen Rollnick, PhD define MI as a 'client centered, directive method for enhancing an intrinsic motivation for change by exploring and resolving ambivalence\". This collaborative approach to working with clients has wide application in coaching, mental health, addictions, health care and health education. Information in this newsletter is taken from various source material from the Motivational Interviewing Network of Trainers and specifically from the book Motivational Interviewing, Preparing People for Change; Miller and Rollnick, 2nd edition, 2002, Guilford Press.
MORE ON AMBIVALENCE
NOTES FROM THE MOTIVATIONAL INTERVIEWING NETWORK OF TRAINERS MEETING--AMSTERDAM 9-06
My MI colleagues, Tom Barth and Christina Nasholm presented a way of looking at ambivalence and Tom posed some interesting some provocative thoughts on working with clients from the MI perspective. I'll attempt to convey them as they connect.
Ambivalence is by no means an unusual state of being. Right now, as I write this, I am ambivalent about dinner: what to have, how to get it... not a terribly large dilemma, a minor thing to have more than one thought about, and how it gets resolved of no great consequence to me.
However, let's say I have come to you to talk about my use of alcohol. From the MI perspective, what I decide to do about this, whether to make a change or not, remains my choice. But, since you will use various MI ways of having me look at alcohol use and by virtue of at least my talking about this, I am likely to find myself considering some change. Not a bad thing, true. But should we warn others that talking may lead to change? It is the putting our thoughts into words and considering alternatives that moves us toward change.
Tom and Christina use an exercise that you can use with clients on matters of ambivalence: Conceptualize the matter of ambivalence as a road that has a name and have the client put a name to the road: how much should I drink? Have the person talk about the issue for a few minutes while you listen and then summarize what you heard. Now, ask the person to think about how far along the road they currently are and how close to making a change/if any they are. Remind the person that there are two choices on the road: change and no change and have the person discuss each direction.
Here is where you can use all of your MI tools for eliciting change talk, remembering to stop and reflect, use summaries and reinforce that the choice remains with the person.
Remember, if YOU decide what choice the client should make, you are likely to begin directing, lecturing, and moving away from the belief in client autonomy.
Keep practicing!
