Welcome to Motivation News, a newsletter devoted to clinical information on the use of a Motivational Interviewing (MI) approach to change. The authors of this approach, William Miller, PhD and Stephen Rollnick, PhD define MI as a 'client centered, directive method for enhancing an intrinsic motivation for change by exploring and resolving ambivalence\". This collaborative approach to working with clients has wide application in coaching, mental health, addictions, health care and health education. Information in this newsletter is taken from various source material from the Motivational Interviewing Network of Trainers and specifically from the book Motivational Interviewing, Preparing People for Change; Miller and Rollnick, 2nd edition, 2002, Guilford Press.
ENHANCING CONFIDENCE--STILL A MATTER OF AMBIVALENCE
'Motivational Interviewing (MI) is a directive, person-centered clinical method for helping clients resolve ambivalence and move ahead with change. It can be applied as a preparation for treatment, a freestanding brief intervention, an enduring clinical style, or a fall- back approach when motivational obstacles are encountered.' Miller and Rollnick, Journal of Cognitive Psychotherapy: An International Quarterly, Vl 18, *4, 2004
The above excerpt is from the article 'Talking Oneself Into Change: Motivational Interviewing, Stages of Change, and Therapeutic Process.'
Rarely do any of us make a decision about a change without some ongoing mixed feelings about it and conversely, most of us don't feel totally incapable. Using an MI style, we can continue to elicit reasons for change and strengthen them. Continuing to use methods for eliciting change talk, brainstorming ideas, reviewing past successes, and giving information and advice are among the ways to elicit and strengthen change talk.
But, a warning, there are some things for the interviewer to avoid: taking responsibility and providing a prescription (the opposite of eliciting/ collaboration) Not taking the issue of confidence seriously enough and missing key elements of self-efficacy And on the other side, continuing to be optimistic about change
Let's return to Jane Doe from last month's MI News.
Jane Doe has been discussing her overall health concerns with the health educator at her medical clinic. She began by talking about upset she is about her weight gain of 20 pounds over the last year.
We'll assume that the interview has progressed. The following summary was provided with the first idea of moving to a change plan, but strengthening confidence comes first. I is for interviewer; J is for Jane.
I; 'So let's see where you stand, from what we have discussed: You really want to get this weight off and keep it off this time (strong Desire). When you look at whether you have what it takes (Ability), you realize you have been finding ways to exercise at least 30 minutes 3-4 times a week and if you can to that, you can lose weight also. Reasons you have cited are better health status, look better, role model for your family, increased energy. You know you Need to do this because of your new diabetic status and your mild difficulty with hypertension. So, where does that leave you?
J: 'I am still committed to doing this but I am still a little doubtful that I can.'
I: 'Okay, that makes sense; you have been successful at an exercise routine; how did you manage that?' (exploring successes)
J: 'Hmm, how did I do that? Well, I decided what was a reasonable goal, looked at my schedule and found the time I could do it. And come to think of it, I made sure I had a way to do exercise at another time or in another way if the first plan would not work.'
I: 'You made a specific plan with flexibility' (simple reflection) And, what else?
J: 'I also made a commitment to others, in that I told them my plan and asked one friend to be my support coach.'
I: 'You are accountable to more than just yourself; you upped the ante so to speak' (complex reflection). How can you use that success to help you with this one?'
J: 'I need to first make sure I have a clear plan for how I am going to eat, with a way I can have treats, not feel deprived.'
I: 'Tell me your ideas thus far; we can brainstorm what will work best for you.'
And so the dialogue continues to a change plan and solidifying commitment. To learn more on Enhancing Confidence, read Chapter 9 of Motivational Interviewing, Preparing People for Change; Miller and Rollnick, 2nd edition, 2002, Guilford Press.
