Welcome to Motivation News, a newsletter devoted to clinical information on the use of a Motivational Interviewing (MI) approach to change. The authors of this approach, William Miller, PhD and Stephen Rollnick, PhD define MI as a 'client centered, directive method for enhancing an intrinsic motivation for change by exploring and resolving ambivalence\". This collaborative approach to working with clients has wide application in coaching, mental health, addictions, health care and health education. Information in this newsletter is taken from various source material from the Motivational Interviewing Network of Trainers and specifically from the book Motivational Interviewing, Preparing People for Change; Miller and Rollnick, 2nd edition, 2002, Guilford Press.
DARN + C REVISITED
'Motivational Interviewing (MI) is a directive, person-centered clinical method for helping clients resolve ambivalence and move ahead with change. It can be applied as a preparation for treatment, a freestanding brief intervention, an enduring clinical style, or a fall- back approach when motivational obstacles are encountered.' Miller and Rollnick, Journal of Cognitive Psychotherapy: An International Quarterly, Vl 18, *4, 2004
The above excerpt is from the article 'Talking Oneself Into Change: Motivational Interviewing, Stages of Change, and Therapeutic Process.'
DARN+C, which I have discussed before is reviewed regarding the interactions of the parts of change. Broken down, D is Desire, A is Ability, R is Reason, and N is Need. If you focus on these elements in eliciting change talk, you can help a client move along the path toward change and increase the likelihood of Commitment talk which then leads to Behavior Change. If there is no Commitment talk, it highly unlikely any change will occur.
Let's return to Jane Doe from last month's MI News.
Jane Doe has been discussing her overall health concerns with the health educator at her medical clinic. She began by talking about upset she is about her weight gain of 20 pounds over the last year.
We'll assume that the interview has progressed. J is client; I is interviewer. This could also be a way to summarize before moving to Stage 2 of MI, which we will discuss more next month.
I; 'So let's see where you stand, from what we have discussed: You really want to get this weight off and keep it off this time (strong Desire). When you look at whether you have what it takes (Ability), you realize you have been finding ways to exercise at least 30 minutes 3-4 times a week and if you can to that, you can lose weight also. Reasons you have cited are better health status, look better, role model for your family, increased energy. You know you Need to do this because of your new diabetic status and your mild difficulty with hypertension. So, where does that leave you?
J: 'I am going to do this and I want us to talk about the various ideas you let me read about last week. Before I leave today, I will make a plan on how to start and set a reasonable goal for myself. (Strong Commitment).
And so the dialogue continues to a change plan and solidifying commitment. Remember the commitment language has to be strong at the end of the interview for the best predictor of behavior change.
