Welcome to Motivation News, a newsletter devoted to clinical information on the use of a Motivational Interviewing (MI) approach to change. The authors of this approach, William Miller, PhD and Stephen Rollnick, PhD define MI as a 'client centered, directive method for enhancing an intrinsic motivation for change by exploring and resolving ambivalence\". This collaborative approach to working with clients has wide application in coaching, mental health, addictions, health care and health education. Information in this newsletter is taken from various source material from the Motivational Interviewing Network of Trainers and specifically from the book Motivational Interviewing, Preparing People for Change; Miller and Rollnick, 2nd edition, 2002, Guilford Press.
AND JUST WHAT IS THIS CONCEPT CALLED SPIRIT? Last month we looked at the MI Principle of Empathy, the effort the interviewer makes to fully understand the client's perspective or view of a problem/concern. (See http://www.cathycoletraining.com for March 05 MI News)
Now let's add MI Spirit which involves collaboration, evocation, and autonomy. Collaboration covers working with/alongside the client in the change process, essentially 'how can we tackle this together?' Evocation means eliciting ideas/change talk from the client: 'what ideas do you have about solving this?' Autonomy reinforces that only the client can decide and has the right to decide.
Let's return to Jane Doe from last month's MI News.
Jane Doe is discussing her overall health concerns with the health educator at her medical clinic. She begins by talking about upset she is about her weight gain of 20 pounds over the last year.
It may go like this: J is client; I is interviewer
J: 'I just can't stand myself with this extra weight!'
I: 'You are really upset about this weight. (reflection) Tell me more.' (open question/evocation)
J: 'I just don't know if I have what it takes to lose weight; can't you just tell me what to do?'
I: 'You're really discouraged. (reflection) What ways have you thought about going about losing weight?' (evocation)
J: 'I doubt if any of my ideas will work; can't you just give me a plan?'
I: 'I can certainly tell you about different ways to go about losing weight and we can discuss how each might work or not for you.' (collaboration) 'Only you can decide how best to go about this plan for you.' (autonomy)
J: 'That might help; I need lots of structure and encouragement'.
I: 'Any plan you decide on must fit what works best for you.' (autonomy) 'We can start by looking at this material together and then talk about the various ways, how you can fit this to work for you.' (collaboration)
And so the dialogue continues to a change plan and commitment.
