Welcome to Motivation News, a newsletter devoted to clinical information on the use of a Motivational Interviewing (MI) approach to change. The authors of this approach, William Miller, PhD and Stephen Rollnick, PhD define MI as a 'client centered, directive method for enhancing an intrinsic motivation for change by exploring and resolving ambivalence\". This collaborative approach to working with clients has wide application in coaching, mental health, addictions, health care and health education. Information in this newsletter is taken from various source material from the Motivational Interviewing Network of Trainers and specifically from the book Motivational Interviewing, Preparing People for Change; Miller and Rollnick, 2nd edition, 2002, Guilford Press.
WHAT IS THIS PRINCIPLE OF EMPATHY? Last month we looked at Premature Focus as something to avoid. (See http://www.cathycoletraining.com for Feb.05 MI News) Essentially, this trap means we, as the counselor, can decide on the topic of importance for a client based on how the client begins the discussion on a problem/concern and completely miss the mark or not determine what the client deems the most pressing place to begin. If we are making the decision about the area of focus, what is important, we are certainly going to create resistance.
How does this fit with the MI Principle of Expressing Empathy? Empathy is the effort the interviewer makes to fully understand the client's perspective or view of a problem/concern. It means that the interviewer places his/her own point of view to the side and uses open questions and reflections to assist the client in fully exploring and developing understanding of the problem, allowing for clarification of ambivalence and for client generated change talk. For some, empathic listening is all that is needed for the client to resolve ambivalence. Let's return to Jane Doe from last month's MI News.
Jane Doe is discussing her overall health concerns with the health educator at her medical clinic. She begins by talking about upset she is about her weight gain of 20 pounds over the last year.
It may go like this: J is client; I is interviewer
J: 'I just can't stand myself with this extra weight!'
I: 'You are really upset about this weight. (reflection) Tell me more.' (open question)
J: 'My mother started out this way at may age, 42 and she just got bigger and bigger, really obese; I can just see myself heading down that path!'
I: 'You don't see yourself as having a choice.' (reflection)
J: 'It feels that way, like I have no way out of this; but I have tackled other big things and been successful.'
I: 'On the one hand you think there is no hope and yet you know you have what it takes.' (reflection)
J: 'Yes, I was able to quit drinking and smoking cigarettes for over 5 years now; it wasn't easy and still I did it. When I think about it, my mother just didn't do anything, try in any way.'
I: 'Stopping drinking and smoking! That's a big achievement for you!' (affirming) And, you realize that you can take action where your mother did not.' (reflection)
J: 'I can tackle this; I just have to find a way that will work for me.'
I: 'You believe in yourself. (reflection). What ways have you considered?' (Open question)
And so the dialogue continues to a change plan and commitment.

Onsite workshops: one to three day training in MI for agencies or educational consortiums
Onsite consultation: Preparing your agency for MI. Combining this with specific training for agency staff increases the effectiveness of implementing MI.
Additinal Advanced MI training: 8 one hour teleclasses for active case supervision and focus on MI skills or on site training for staff groups already trained in MI; feedback on tapes using the MITI.
